My experience includes safely managing ​fragile seniors with complex medical and care needs of including those living at home, assisted living, skilled nursing, long-term care, post-acute, and transitional care settings in a shifting healthcare environment.

​As your Expert Adult NP Witness, it is my privilege to offer an objective analysis of the accuracy of diagnosis, review of elements of safe, effective multi-comorbid ​complex disease management, consideration of pharmacokinetics, root cause analysis, accurate and differential diagnosis, use of evidence-based ​and local standards of care, ​evaluation of NP provider competency and the impact of their recommendations within the context of patient outcomes and injury.

Key Qualifications:

Serious Illness (Non-Hospital Critical/Acute Care) & Geriatric Care: Extensive experience in traditional and ACO/Value-Based care for the fragile patient with advanced and end-stage diseases, deploying a comprehensive knowledge of AHCA regulations and PointClickCare​ to evaluate Nurse Practitioner documentation for sentinel events, identifying avoidable versus unavoidable wounds and identification of factors indicating the need to escalate level of care.

Analysis of avoidable sentinel events including but not limited to

• · application of the principles of antibiotic stewardship, identifying criteria for empiric treatment, accurate culture interpretation, and initiation of targeted antibiotic for urinary tract infections​ to avert Sepsis and prevent recurrent urinary tract infections with worsening resistance,

• · review for use of Beers list medications, perform review of systems with pertinent exam to perform plan modification to avert recurrent falls,

• · discerning failure to thrive from failure to feed and hydrate,

• · preventing aspiration pneumonia and fecal impaction by performing an accurate physical exam with detailed record review and initiation of ​appropriate ​empiric management.

Complex Wound Management:

Proficient in performing and interpreting comprehensive wound exam accurately, evaluating NP training for competency and safety considerations for performing unsupervised outpatient surgical procedures, identify appropriate dressings orders in compliance with infection control practices for containment, and confirmation of medical necessity for common outpatient wound procedures including selective and surgical sharp debridement, biopsies, CTP, and cryotherapy.

Definitive and differential diagnosis with standard of care evaluation and management ​of pressure ulcers, multi-drug-resistant infected wounds, diabetic/neuro-ischemic, radiation late effects, vascular, atypical wounds, wounding dermatologic disease, medical device injury, failed CTP/graft, moisture associated skin damage and skin failure.

Offering 3+ Decades of Clinical experience practicing across the healthcare spectrum from Home, Hospital, Post-Acute, Rehab, Long-Term Care settings.

Procedural Proficiency: Skilled in best practices for performing common outpatient surgical wound procedures and invasive device management with documentation of explicit steps to avoid complications (e.g., PEG tube change strategies to prevent concurrent aspiration, PICC lines, tracheostomies, stoma hypergranulation management to avert tube dislodgement and reduce risk of fascia inoculation of bacteria (Fournier's gangrene risk), vascular studies, and conservative surgical interventions
