• No Forced Groups or Therapy

  We treat OUD as a medical condition. While group support definitely helps during the early transitional period, over time many people develop their own support systems and have resumed normal life activities. We may recommend counseling or therapy, but we never require or mandate groups, therapy, reading assignments, or behavioral checklists. Your care is based on a medical management model and what works for you.

• Ready-to-Deploy Backup Plans

  If you have tried to wean before and didn't work out, had your script cut off or not in stock, ended up really miserable, you probably can explain why a backup plan is important better than us. If withdrawal symptoms re-emerge during a taper or even after you jump off altogether several months later, we assure you have pre-agreed backup plan for rapid deployment to protect your brain pathways from getting stuck in the cycle of addiction loop again.

How the Injection Works

Brixadi® and Sublocade® are long-acting buprenorphine injections. The medication is given as a shot just beneath the skin into the fatty tissue layer (above the muscle).

Think of it exactly like a long-acting birth control shot—it forms a small, quiet deposit that slowly dissolves over time, releasing a steady, continuous stream of medication into your system. This keeps your medicine working evenly in the background without daily ups and downs.

How it Helps with Weaning & Tapering

Because the medicine dissolves so smoothly, the amount of buprenorphine in your system naturally and very slowly drifts down over weeks and months. This creates a built-in, microscopic "self-taper." This gradual decline is exactly why long-acting injections are so helpful for individuals who want to comfortably reduce their dose or smoothly transition completely off buprenorphine without the daily hassle of cutting up films. The following doses are decreased based on comfort and response lowering over several months.

What to Expect During Your Injection Appointment

The day is finally here, your local care team called, confirmed, now you've arrived and it is time for your shot, the process is quick, discrete, and managed entirely by the on-site local healthcare or nursing professional.

• Where is it given? Sublocade® is typically given in the abdominal area, while Brixadi® offers more flexible options and can be injected into the stomach, thigh, buttock, or upper arm (after 4 shots).

• What does it look and feel like? The medication forms a small, firm pocket or deposit under the skin. It is not visible through your clothing and will slowly dissolve on its own over time.

• ⚠️ Important Healing Rule: Do not rub, scratch, squeeze, or pick at the injection site. Leave the area completely alone so the medication can settle and dissolve correctly. If you notice redness, swelling, discharge or pain at the site notify us immediately via Your Patient Secure Text, Portal Message or Direct Call.

• Does it hurt? You may feel a brief sting or pinch during the injection, and the area might be slightly tender or red for a couple of days. This is completely normal.

• After the first few shots you may be asked to hang around for about 30 min to make sure you don't have any problems (like with other shots).

• Your Job: Once the injection is completed by your local RN/LPN or care team, your dose starts. You don't have to think about a film, tab, carrying it around, or a pharmacy counter for the rest of the month (or weekly)-- your job is to get on with life. However! If you miss that daily routine of taking Suboxone that's normal-we have some ideas that taste better & may really help!

When Are You Ready for the Injection?

While anyone on films or tablets for 7 days can "technically" qualify for the shot—and some platforms market a fast track to start immediately-InChargeClinic is dedicated to a more cautious, medical-safety first approach. We believe in getting it right the 1st time.

(NOTE: It may take a few weeks to get the ducks in a row (your paperwork, insurance approval, specialty rx processing, coupon approval and local injection site processed)-- so we can begin the process earlier then adjust final prescription to be ready when you are ready.

• Allowing Things to Settle Down First (Common Sense)

  If you recently started Suboxone, just experienced a weaning attempt with breakthrough withdrawal symptoms, or have faced recent health or social challenges and battled some triggers, how much Suboxone you need to control withdrawal symptoms and cravings may change as things settle down. We focus on helping things stabilize first, so we can find the exact dose that works best for you.

• Verifying True Dose Stability

  Once things have leveled out, we recommend waiting until you have been stable on that consistent, predictable daily dose for at least several weeks before making the switch. (It may take a few weeks to get the prescription processed & locate a local injection site anyway)

• The Injection Cannot Be Reduced or Undone after the stick.

  Most shots are monthly. Some weekly to start but for both, the dose cannot be changed or reduced once it is injected and has to wear off. For example: you are prescribed 3 films a day but you are really taking 2. If the monthly shot is calculated from the 3-film prescription — not from what you actually take — the injection amount can be too high. Excessive sleepiness is the usual problem. Once it is in, you cannot take it back.

• A Calculated, Safe Switch

  We focus on getting it right the first time. We wait until your perfect daily dose is completely locked in and proves to be good for a sustained period of time. Once we are absolutely sure of your exact daily requirement, we can safely calculate the correct milligram amount for the injection and convert you to the long-acting buprenorphine.
