For Florida Adults and Seniors who have achieved stability, control over the cycle of addiction with Buprenorphine/Naloxone (Suboxone®) or Long-Acting Injection (Sublocade® or Brixadi®) and are ready for normal routine chronic medication management.
Prerequisites
You have completed Early & Stable MAT Programs or Transferring from a prior MAT outpatient program with documentation of stable long-term use of MAT (we can find on eForce/PDMP), and met the milestones & objectives in the Stable MAT Program.

Long-Term Chronic MAT Program
Buprenorphine/Naloxone or Long-Acting Buprenorphine Treatment has become a routine part of life with ongoing self-management and long term stability.
Eligibility: ≥12 -18 months with documentation of continuous stable MAT with physical, emotional, and social stabilization.
Visits: Individually determined. Generally monthly to bi-monthly unless experiencing dose adjustment or other issues.
Transferring patients with prior stable MAT < 12-18 month are typically see monthly for the first 3–6 months prior graduating to Long-Term/Chronic MAT program.
Virtual Face-2-Face visits are mandatory plus monthly digital/audio check-ins.
Goal 90 Day Program-
Medication is fully integrated into daily life as one of your many reliable tools supporting whole-person stability.
Your status is and has been stable for a long, long time and you require basic chronic medication management just like treating cholesterol or high blood pressure.
Visits: Individually determined after brief monthly trial to get things set-up. Typically quarterly visits + monthly check-ins with audio calls and digital text. All prescriptions require a virtual face-2-face encounter for a legal prescription to be written.
Prescriptions: Based on your pharmacy and/or insurance requirements. Film or Tabs may be monthly partial fills for total of 90 written at the face-2-face quarterly visit or 90-day supply. Injections may be written for monthly for a total of 3 months for approved alternate site location injection.
