Accomplish Milestones: Right Effective Dose, Effective Trigger and Craving Control, Documentation to Credit prior MAT, Commitment-show up and take med as prescribed, meet Terms of Service, learn about options for ongoing care (wean, long-acting buprenorphine, commit to healing)

Next Step- Stable MAT level.

Note: Those who have documented stability on chronic MAT may be placed in Long-Term MAT Program more rapidly.

Appointment Schedule Early/Transitional Program

Scheduling may be individualized but generally---

First visit + required 2-week follow-up (home CLIA or recent lab submitted prior to 2nd visit)

New, resuming, or post-detox: may be weekly or biweekly for about 6–8 weeks

Stable & transferring? may go monthly after the second visit + UDS

Video visit required for every controlled-substance prescription

New, those requesting Safe Taper or converting to Brixadi®* should expect additional interval secure digital/SMS or audio contact.
