Affordable Suboxone Telehealth Florida – Financial Hardship Plan & Pricing

InChargeClinic provides statewide Florida telehealth for Suboxone (buprenorphine/naloxone) treatment of opioid use disorder, alcohol use disorder reduction plan with medication, and interval-limited adult primary care. For those with documented financial hardship, we offer reduced flat-rate pricing through our Income-Based Financial Hardship Plan. We know times are tough.

Financial Hardship Plan Pricing

New Patient Hardship Visit: $65

Follow-Up Hardship Visit: $45

Select the matching visit type (New Hardship Visit or Follow-Up Hardship Visit) during registration. These rates apply only to patients who meet the income limits below and complete all required steps and fill out new patient forms.

Income Limits by Household Size

1 person: gross monthly income under $4,000

2 or more people: gross monthly income under $5,500

Documentation Requirements

To confirm eligibility, the practice may request documentation at any time, such as a recent bank statement, paycheck stub, other proof of income or case management referral. All documents are handled under HIPAA and reviewed only for verification purposes. Access to the program remains discretionary.

Visit fee does not include medication or testing.

We review low-cost options during your visit, including home CLIA testing (~$5–6) and pharmacy discounts.

How to Request Hardship Visit

1. Click Request Appointment Start Registration, pick a day and time.

2. Choose the New Hardship Visit for your first time visit, Select follow-up Hardship for returning if you are a continuity of care previously established patient.

3. Complete the pre-screening form and submit.

4. You will be sent a text and email to join our portal to complete new patient paperwork. You can also complete this from the text or email link.

5. New patient paperwork includes: medical history intake, consent forms, affidavit of uninsured or ok to pay cash form, uploading your state ID or drivers license and payment info.

6. Be prepared to provide income documentation (bank statement, paycheck stub, or other proof) if requested.

7. Once registered we will also send you an invite link to Spruce-our HIPAA secure direct text and call number. You can contact your provider directly and securely upload any images such as State ID/drivers license and home drug test for direct secure communication with your provider.

8. Failure to completely register, fill out forms and upload any requested forms will result in your appointment being delayed.

Services Available Under Hardship Program

• Suboxone / buprenorphine-naloxone medication management for opioid use disorder (new starts, continuation, or supervised taper)

• Induction related (starting Suboxone) comfort medication prescriptions and plan of care.

• Safe Taper Back-Up Plan medication prescriptions

• Lab Orders or recommendations for home CLIA UDS testing

• Alcohol use disorder reduction medication management (example: Naltrexone)

• Interval-limited adult primary care for non-emergent health problems.

• Statewide Florida telehealth only (no in-person visits)
